Abiraterone Acetate falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the simple version. Abiraterone (Yonsa) can be tough on the liver on its own, which is why your oncology team checks your liver blood tests regularly while you're on it. Padang cassia, a cassia-type cinnamon, naturally contains coumarin, and in large or concentrated doses coumarin has been linked to liver irritation. Stacking two things that can stress the liver is the worry here.

To be clear, this concern is mostly theoretical and based on the pharmacology, not strong human studies. A little cinnamon sprinkled on food is very unlikely to matter. The caution is really about concentrated cinnamon supplements taken daily. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or oncologist before adding a cinnamon supplement.

Abiraterone Acetate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the simple version for your prostate cancer medicine, abiraterone (Yonsa). Your liver uses an enzyme called CYP3A4 to help process this drug. In lab tests only, a concentrated extract of Padang cassia cinnamon appeared to slow that same enzyme down. If that were to happen in real life, abiraterone could build up a bit more than expected, which might mean stronger effects and a higher chance of side effects like high blood pressure, swelling in your legs, low potassium, fatigue, or liver enzyme changes.

I want to be honest though: this is only a test-tube finding, not proven in people, so it may not matter much. Sprinkling cinnamon on food is not a concern. Before starting a concentrated cinnamon supplement, just run it by your oncology team or pharmacist first.

About Abiraterone

What Is Abiraterone Used For?

Brand names: Zytiga, Yonsa

Abiraterone acetate tablets are used to treat two types of advanced prostate cancer: metastatic castration-resistant prostate cancer (CRPC — cancer that has spread and keeps growing despite low testosterone levels) and metastatic high-risk castration-sensitive prostate cancer (CSPC — cancer that has spread but still responds to hormone-lowering treatment). It is always taken together with a corticosteroid — most products pair with prednisone, while Yonsa is specifically indicated with methylprednisolone.

All products — including Abiraterone, Abiraterone Acetate, Abirtega, Yonsa, and Zytiga — are taken by mouth as a tablet. Yonsa is approved only for metastatic CRPC, while the other products (Abiraterone, Abiraterone Acetate, Abirtega, and Zytiga) are approved for both metastatic CRPC and metastatic high-risk CSPC.
