Acetaminophen, Caffeine, Dihydrocodeine falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the thing to keep an eye on with this combination: the acetaminophen part of your Panlor or DHC Plus is processed by your liver, and in large amounts it can stress that organ. Padang cassia is a cassia-type cinnamon that naturally contains coumarin, a compound that has been linked to liver irritation when taken in concentrated, high doses over time. Stacking two things that lean on your liver is the concern here.

To be clear, this is a theoretical caution based on how these compounds behave, not a well-documented event in real patients. Sprinkling cinnamon on your oatmeal is no problem. The worry is concentrated cinnamon capsules, especially if you have any liver disease or drink alcohol. If you're considering a cinnamon supplement, just run it by your pharmacist or doctor first.

Acetaminophen, Caffeine, Dihydrocodeine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist with this combination pill (acetaminophen, caffeine, and dihydrocodeine). The dihydrocodeine part leans on liver enzymes, including CYP3A4, to be processed. Padang cassia, in test-tube studies only, slowed down CYP3A4. In theory, that could let dihydrocodeine and caffeine hang around a bit longer, which might nudge up effects like drowsiness, dizziness, constipation, nausea, or feeling jittery from the caffeine.

I want to be honest though: this is based only on lab data, not on real people, so it's quite uncertain. Sprinkling cinnamon on your food is no worry. The bigger question is concentrated cinnamon capsules, which also carry coumarin that can stress the liver, something acetaminophen already taxes. Please check with your pharmacist or doctor before adding a cinnamon supplement.
