Acetaminophen, Pseudoephedrine, Triprolidine falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the simple version. The acetaminophen (Tylenol) part of Actifed Plus ES is processed by your liver, and in high doses or with heavy alcohol use it can stress the liver. Concentrated Padang cassia (cassia cinnamon) supplements contain a natural compound called coumarin, which in large amounts has been linked to liver trouble too. The worry is that stacking two things that lean on the liver could add up in sensitive people.

To be clear, this is theoretical and based on how these compounds behave, not on strong human studies. Cinnamon used as a kitchen spice is not a concern. The bigger issue is taking concentrated cinnamon capsules plus more acetaminophen than directed. Stay within label dosing and check with your pharmacist if you take cinnamon supplements regularly or have any liver concerns.
