Padang Cassia and Alectinib Hydrochloride

Here is the interaction we found between these two, with the evidence behind it.

Moderate

HEPATOTOXIC DRUGS

Why this applies to Alectinib Hydrochloride

Alectinib Hydrochloride falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind with alectinib (Alecensa): this cancer medicine can already be tough on your liver. In fact, your oncology team likely checks your liver enzymes regularly while you're on it, because liver inflammation is a known side effect. Concentrated cinnamon supplements like Padang cassia contain a natural compound called coumarin, which in large or daily amounts has been linked to liver strain in sensitive people.

So the worry isn't that cinnamon changes how alectinib works. It's that stacking two things that can stress the liver may add up. This concern is mostly theoretical, based on how coumarin behaves, not on strong human studies with this exact pairing. Sprinkling cinnamon on your oatmeal is fine. Before taking concentrated cinnamon capsules, please check with your pharmacist or oncologist.

From our pharmacist’s overview on the Alectinib Hydrochloride page

“Alectinib (Alecensa) treats ALK-positive lung cancer, and because your liver already has to tolerate this drug, check this page before adding any supplement, not after. Of the 74 ingredients on file, 38 can add stress to the liver, and three of the four major-rated entries (beer, wine, and Siberian cocklebur) are the clearest examples…”

About Padang Cassia

What is Padang Cassia?

  • Adding flavor to food
  • Digestive upset
  • Blood sugar support
  • Antimicrobial purposes
  • Antioxidant support

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.
