Benzhydrocodone, Acetaminophen falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the thing to keep an eye on with Apadaz (benzhydrocodone and acetaminophen): the acetaminophen part is processed by your liver, and in high doses it's one of the more common causes of liver strain. Padang cassia is a cassia-type cinnamon that naturally contains coumarin, which in large, concentrated amounts has been linked to liver irritation. So the worry is simply two things that can tax the liver stacking on top of each other.

To be honest, this concern is theoretical and based mostly on how these compounds behave, not on strong human studies. A sprinkle of cinnamon on your oatmeal is no problem. Concentrated cinnamon capsules taken daily are a different story. Please check with your pharmacist or doctor before pairing them, especially if you have any liver issues or drink alcohol regularly.

Benzhydrocodone, Acetaminophen also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the thing to know about Apadaz (benzhydrocodone with acetaminophen): the benzhydrocodone part is actually a prodrug. Your body has to convert it into hydrocodone, the active painkiller, and enzymes like CYP3A4 are involved in processing it further. Lab studies suggest concentrated Padang cassia (a cassia cinnamon) may slow down CYP3A4. In theory, that could let more opioid activity build up, leading to stronger effects like drowsiness, dizziness, constipation, nausea, or in serious cases slowed breathing.

I want to be honest though: this is based only on test-tube data, not real people, so the risk is uncertain. Sprinkling cinnamon on your oatmeal is not a concern. Concentrated cinnamon capsules are where I'd pause. Please check with your pharmacist or doctor before combining them.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

What Is Benzhydrocodone and Acetaminophen Used For?

Brand name: Apadaz

Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.

Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
