Drospirenone, Estetrol falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the simple version. Nextstellis is a birth control pill that combines drospirenone with estetrol, a newer estrogen. Like most hormonal birth control, it's processed by your liver, and on rare occasions these pills can cause mild liver-related effects. Padang cassia is a type of cassia cinnamon that naturally contains coumarin, which in large amounts has been linked to liver strain. The worry here is simply two things that lean on your liver stacking up.

To be clear, this is a theoretical concern based on how these substances behave, not proven harm. Sprinkling cinnamon on your oatmeal won't matter. The caution is really about concentrated cinnamon supplements taken daily, especially if you have any liver issues. Before starting one alongside Nextstellis, just run it by your pharmacist or doctor.

Drospirenone, Estetrol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Nextstellis combines two active hormones, drospirenone and estetrol, and your body partly clears them using a liver enzyme called CYP3A4. In a test-tube study, a concentrated extract of Padang cassia slowed this enzyme down. If that happened in real life, the hormones could linger a bit longer and you might notice more of the usual birth control side effects, like breast tenderness, nausea, headaches, or mood changes.

Keep this in perspective. This is only lab data, not something proven in people, and sprinkling cinnamon on your oatmeal is very unlikely to do anything. The bigger question is concentrated cinnamon capsules. If you are thinking about taking those regularly with your pill, run it by your pharmacist or doctor first.
