Padang Cassia and Infigratinib

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Infigratinib

Infigratinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Padang Cassia interacts with.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Truseltiq (infigratinib): your body clears this cancer medicine largely through a liver enzyme called CYP3A4. Lab tests suggest a concentrated extract of Padang cassia (a cassia-type cinnamon) can slow that same enzyme down. If that happened in a real person, infigratinib could build up a bit more than expected and push its side effects harder, things like high phosphate levels, eye and vision changes, mouth sores, dry eyes, diarrhea, or fatigue.

I want to keep this in perspective. This is only a test-tube finding, not something proven in people, and sprinkling cinnamon on your oatmeal is very unlikely to matter. Concentrated cinnamon capsules are a different story. Since infigratinib is a serious drug, please run any supplement past your oncologist or pharmacist first.

From our pharmacist’s overview on the Infigratinib page

“Infigratinib (Truseltiq) treats a form of bile duct cancer, and the supplements on this list deserve genuine care, because most of them can shift how much of the drug actually reaches your blood. Nearly all of these interactions run through CYP3A4, the liver enzyme that breaks infigratinib down.”

About Padang Cassia

What is Padang Cassia?

  • Adding flavor to food
  • Digestive upset
  • Blood sugar support
  • Antimicrobial purposes
  • Antioxidant support

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

About Infigratinib

What Is Infigratinib Used For?

Brand name: Truseltiq

  • Infigratinib is used in adults who have received previous therapy to treat a certain type of cholangiocarcinoma (bile duct cancer) that has spread to nearby tissues or other parts of the body and cannot be removed by surgery. Infigratinib is in a class of medications called kinase inhibitors. It works by blocking the action of the abnormal protein that signals cancer cells to multiply. This helps stop or slow the spread of cancer cells.
