Here's the thing about lomitapide (Juxtapid): it's already pretty hard on the liver. In fact, it carries a serious warning about liver injury, and people on it get regular blood tests to watch their liver enzymes. That's why this combination is worth a second look.
Padang cassia is a cassia-type cinnamon, and it naturally contains coumarin, which in large amounts has caused liver problems. The concern here is simple: stacking a liver-stressing supplement on top of a liver-stressing medication may add up. To be fair, this is a theoretical worry based on how these compounds behave, not proven cases. Sprinkling cinnamon on your oatmeal isn't the issue. Concentrated cinnamon capsules are. Before taking any cinnamon supplement, please check with your doctor or pharmacist first.

Here's the gist for lomitapide (Juxtapid): your body relies heavily on an enzyme called CYP3A4 to clear this drug. In lab dish studies, a concentrated extract of Padang cassia cinnamon appeared to slow that enzyme down. If that happened in real life, lomitapide could build up in your blood and hit harder than intended.
Lomitapide isn't a prodrug, so more drug means stronger effects and more side effects, mainly stomach trouble like diarrhea, nausea, and cramping, plus a real concern for liver strain (rising liver enzymes and fat buildup in the liver). I'll be honest, this is only theoretical and based on lab work, not people. Still, lomitapide is a drug doctors watch closely, and cinnamon supplements add their own coumarin liver risk. Sprinkling it on food is fine, but check with your pharmacist before any cinnamon capsules.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. What Is Lomitapide Used For?
Brand name: Juxtapid
Juxtapid (lomitapide) is used alongside a low-fat diet and other cholesterol-lowering treatments — including LDL apheresis (a procedure that filters LDL from the blood) where available — to lower LDL cholesterol, total cholesterol, apolipoprotein B, and non-HDL cholesterol in patients with homozygous familial hypercholesterolemia (HoFH). HoFH is a rare, inherited condition that causes extremely high LDL cholesterol levels from birth.
Juxtapid is approved for use in adults and in children aged 2 years and older with HoFH. It has not been proven safe or effective for other types of high cholesterol, including the more common heterozygous familial hypercholesterolemia (HeFH). Its effect on heart attacks, strokes, or other cardiovascular events has not been established.
