Lorlatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Padang Cassia interacts with.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist for Lorbrena (lorlatinib): your liver uses an enzyme called CYP3A4 to break this medicine down. Lab studies suggest a concentrated extract of Padang cassia can slow that enzyme down. If that happened in your body, lorlatinib could clear more slowly and build up, which may bump up its effects and side effects. With lorlatinib that could mean things like higher cholesterol or triglycerides, swelling, mood or thinking changes, slowed heartbeat, or fatigue.

Now, the honest part: this is only test-tube evidence, not proven in people, and sprinkling cinnamon on your oatmeal is very unlikely to matter. Concentrated cinnamon supplements are a different story. Since lorlatinib is a serious cancer drug, please check with your oncology team or pharmacist before adding any cinnamon supplement.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

What Is Lorlatinib Used For?

Brand name: Lorbrena

Lorbrena (lorlatinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — meaning lung cancer that has spread — when lab testing confirms the tumor is ALK-positive (has a specific gene change that drives cancer growth). Patients must be tested with an FDA-approved diagnostic test before starting treatment.

Lorbrena has been studied in patients who have not yet received treatment for advanced disease, as well as in patients whose cancer progressed on other ALK-targeting medicines.
