Here's the concern with pairing concentrated cinnamon supplements (like Padang cassia) with Nerlynx (neratinib). Cassia cinnamon naturally contains a compound called coumarin, and in large supplement doses coumarin has been linked to liver stress. Neratinib itself can already be tough on the liver, so doctors monitor your liver enzymes during treatment. Stacking another potential liver irritant on top is the worry here.
This is a theoretical caution based on how these compounds behave, not proven harm in people on neratinib, so don't panic. The amount of cinnamon you sprinkle on food or in your coffee is very unlikely to matter. It's the concentrated capsule doses I'd want you to clear with your oncologist or pharmacist first, since they're already watching your liver closely.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Here's the simple version: your body relies on a liver enzyme called CYP3A4 to break down neratinib (Nerlynx). Lab tests suggest a concentrated extract of Padang cassia can slow that enzyme down. If it does, neratinib could clear more slowly and build up in your system, which may push up its effects and its side effects. With neratinib that often means worse diarrhea (a very common and sometimes severe issue with this drug), nausea, stomach upset, or changes in your liver tests.
Now the reassuring part: this is only a test-tube finding, not something proven in people, and sprinkling cinnamon on your food is very unlikely to matter. Concentrated cinnamon capsules are a different story. Since neratinib is a serious cancer medicine, please check with your oncologist or pharmacist before adding any cinnamon supplement.

What Is Neratinib Used For?
Brand name: Nerlynx
Nerlynx (neratinib) is used on its own as extended adjuvant treatment — meaning ongoing therapy after initial treatment is done — for adults with early-stage HER2-positive breast cancer who have already completed trastuzumab-based therapy.
Nerlynx is also used in combination with another chemotherapy drug, capecitabine, for adults with advanced or metastatic (spread) HER2-positive breast cancer who have already received two or more prior HER2-targeted regimens in the metastatic setting.
