Pralsetinib falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the concern with combining concentrated cinnamon supplements like Padang Cassia and your cancer medication Pralsetinib (Gavreto). This isn't about cinnamon changing how your body processes the drug. Instead, both can be tough on the liver. Cassia cinnamon naturally contains coumarin, which in large supplement doses has been linked to liver injury. Pralsetinib itself can raise liver enzymes and, less often, cause more serious liver problems, so stacking another liver stressor on top isn't ideal.

To be clear, this is a theoretical worry based on how these compounds behave, not strong human proof for this exact pair. Sprinkling cinnamon on your oatmeal is unlikely to matter. The caution is for daily high-dose cinnamon capsules. Since your care team already monitors your liver while you're on Pralsetinib, please check with them before adding any cinnamon supplement.

Pralsetinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the simple version: your body uses an enzyme called CYP3A4 to clear pralsetinib (Gavreto) out of your system. Lab studies hint that concentrated Padang cassia cinnamon may slow that enzyme down a bit. If that happened in real life, pralsetinib could build up and its effects, along with its side effects, might get stronger. With this drug that could mean more fatigue, high blood pressure, swelling, mouth sores, constipation, or changes in blood counts.

I want to be honest about how weak this evidence is. It comes only from a test-tube study, not from people, so we don't really know if it matters at human doses. Cinnamon used in cooking is very unlikely to cause trouble. Before starting a concentrated cinnamon supplement, just run it by your oncology team or pharmacist.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

What Is Pralsetinib Used For?

Brand name: Gavreto

Gavreto (pralsetinib) is used to treat adults with metastatic RET fusion-positive non-small cell lung cancer (NSCLC) — a type of lung cancer that has spread and carries a specific genetic alteration detected by an FDA-approved test.

Gavreto is also approved for adults and children aged 12 and older with advanced or metastatic RET fusion-positive thyroid cancer who need systemic (whole-body) treatment and whose cancer no longer responds to radioactive iodine (when radioactive iodine is an appropriate option). This thyroid cancer use is under accelerated approval, meaning continued approval depends on results from confirmatory clinical trials.
