Voriconazole falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the gist for voriconazole (Vfend): this isn't about the spice changing how the drug works in your body. It's about the liver. Voriconazole is already known to be tough on the liver, and your care team usually checks liver enzymes while you're on it. Padang cassia is a cassia-type cinnamon that contains coumarin, which in large amounts has been linked to liver irritation.

So the worry is doubling up two things that can stress the liver. Honestly, this is a theoretical concern based on how coumarin behaves, not strong human proof. A sprinkle of cinnamon on your food is very unlikely to matter. Concentrated cinnamon capsules are a different story. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before taking any cinnamon supplement.

Voriconazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist with voriconazole (Vfend). Your liver uses an enzyme called CYP3A4 to help clear this antifungal from your body. Lab studies suggest that concentrated Padang cassia cinnamon may slow that enzyme down a bit. In theory, that could let voriconazole linger longer and build up, which may bump up its effects and side effects, things like visual changes (blurry vision or seeing brighter light), nausea, headache, or liver irritation.

I want to be honest about the strength here: this is based only on test-tube research, not real patients, so it's far from certain. Sprinkling cinnamon on your oatmeal is no worry. If you're thinking about a concentrated cinnamon supplement while on voriconazole, please run it by your pharmacist or doctor first.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

About Voriconazole

What Is Voriconazole Used For?

Brand name: Vfend

Voriconazole treats several serious fungal infections. Across all available forms — tablets, oral suspension, and intravenous (IV) injection — it is used for invasive aspergillosis (a dangerous mold infection), candidemia (Candida fungus in the bloodstream) in patients who are not neutropenic (not severely low on white blood cells), other deep-tissue Candida infections, and serious infections caused by Scedosporium apiospermum or Fusarium species in patients who cannot tolerate or have not responded to other antifungal treatments.

Esophageal candidiasis (a Candida infection of the esophagus, the swallowing tube) is also an approved use for most formulations. Note on the oral tablet form (generic voriconazole tablets): that specific tablet formulation is approved for patients aged 12 to 14 years weighing at least 50 kg, and all patients 15 years and older — not for younger or smaller children. The oral suspension and the IV injection are approved for patients 2 years and older for most of these conditions.
