Tenapanor Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Passion Flower interacts with.

Theoretically, passion flower might decrease the effects of CYP3A4 substrates.
In vitro research suggests that passion flower can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the good news if you take Ibsrela (tenapanor) for IBS with constipation: this interaction is mostly theoretical and very minor. The concern on file is that passion flower might slightly speed up a liver enzyme called CYP3A4, which could in theory lower levels of drugs that enzyme processes. In practice, this matters less for tenapanor than for many other medicines, because tenapanor works locally in your gut and very little of it is actually absorbed into your bloodstream.

The evidence here comes only from lab studies, not real people, and the effect was far weaker than known enzyme boosters. So I wouldn't expect passion flower to noticeably change how well your tenapanor controls bloating or bowel movements. Still, it's worth a quick chat with your pharmacist or doctor before combining them.

Tenapanor is approved for two distinct conditions, each under a separate brand. Ibsrela is used to treat irritable bowel syndrome with constipation (IBS-C) in adults. XPHOZAH is used to reduce high phosphorus levels (hyperphosphatemia) in adults with chronic kidney disease (CKD) who are on dialysis — specifically as an add-on when phosphate binders haven't worked well enough or aren't tolerated.
