Mexiletine falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Pu-erh Tea interacts with.
Pu-erh tea contains caffeine. Caffeine is metabolized by CYP1A2. Theoretically, drugs that inhibit CYP1A2 may decrease the clearance rate of caffeine from pu-erh tea and increase caffeine levels.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind with this pairing: mexiletine actually blocks an enzyme called CYP1A2, and that same enzyme is what your body uses to clear caffeine. Pu-erh tea has caffeine in it. So when the two are combined, the caffeine can hang around longer than usual and build up a bit more than you'd expect.
In plain terms, your usual cup might hit harder. You could notice things like jitteriness, a racing or pounding heartbeat, trouble sleeping, or feeling on edge. That's worth a thought since mexiletine is a heart rhythm medicine and you don't want extra palpitations muddying the waters. This is a theoretical concern based on how the drugs work, not strong human proof, and a normal cup or two is usually manageable. Still, mention your tea habit to your pharmacist or doctor so they can keep an eye on it.

Here's the thing to know about this combo: it's not that pu-erh tea changes how your mexiletine works. It's actually the other way around. Mexiletine slows down how fast your body clears caffeine, and pu-erh tea contains caffeine. Studies suggest mexiletine can cut caffeine elimination by about half, so the caffeine simply hangs around longer.
In real life, that means the cup of pu-erh you drink might hit you harder than usual. You may notice things like jitteriness, a racing or pounding heart, trouble sleeping, or feeling wired. This is a minor, possible interaction, not a dangerous one, but if you drink a lot of caffeinated tea, mention it to your pharmacist or doctor so you can adjust comfortably.

Pu-erh tea is a fermented, aged form of tea made from the same plant as green and black tea. It is enjoyed worldwide as a beverage and is sometimes promoted for weight loss, cholesterol, and digestion, but human evidence for these uses is limited. As a normal drink it is generally well tolerated, though it does contain caffeine.
About Mexiletine
What Is Mexiletine Used For?
Brand name: Mexitil
Mexiletine Hydrochloride capsules are indicated for the treatment of documented, life-threatening ventricular arrhythmias — for example, sustained ventricular tachycardia (a dangerously fast heart rhythm originating in the lower chambers). Because mexiletine itself can trigger arrhythmias, it is not recommended for minor or symptom-free irregular heartbeats.
Treatment is started in a hospital setting so heart rhythm can be monitored closely. Antiarrhythmic drugs, including mexiletine, have not been shown to improve overall survival in patients with ventricular arrhythmias.
