Pu-erh Tea and Ephedrine, Hydroxyzine, Theophylline

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Pu-erh Tea interacts with whole categories of medication. Ephedrine, Hydroxyzine, Theophylline falls into 3 of those categories, so it can interact with Pu-erh Tea in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Major).

Major

EPHEDRINE

1 of 3
Theoretically, concomitant use might increase the risk for simulant adverse effects.
Pu-erh tea contains caffeine. There is evidence that using ephedrine with caffeine might increase the risk of serious life-threatening or debilitating adverse effects such as hypertension, myocardial infarction, stroke, seizures, and death.
Severity Major Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Here's the thing to know about this combination product: it already contains ephedrine (a stimulant) and theophylline (which is closely related to caffeine and also revs up the body). Pu-erh tea adds even more caffeine on top of that. Stacking stimulants together can push your heart rate and blood pressure higher, leave you jittery or unable to sleep, and in more serious cases raise the risk of a racing or irregular heartbeat, chest pain, or seizures.

Theophylline in particular has a narrow safety window, so even modest extra stimulation matters. The evidence here is mostly anecdotal, but the concern is real enough that I'd keep tea intake modest and talk with your pharmacist or doctor before relying on pu-erh regularly with this medication.

STIMULANT DRUGS

2 of 3

Why this applies to Ephedrine, Hydroxyzine, Theophylline

Ephedrine, Hydroxyzine, Theophylline falls under STIMULANT DRUGS — the group Pu-erh Tea interacts with.

Theoretically, concomitant use might increase stimulant adverse effects.
Pu-erh tea contains caffeine. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Severity Moderate Likelihood Probable Evidence C

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: this combination medicine already contains two stimulants, ephedrine and theophylline. Pu-erh tea brings its own caffeine to the party. Stacking caffeine on top of these can pile up the stimulant effects, so you might notice a faster or pounding heartbeat, jitteriness, trouble sleeping, anxiety, or an upset stomach. Theophylline in particular has a narrow safety window, so I pay extra attention there.

This concern is mostly based on expert reasoning rather than strong studies, so don't panic. A single cup now and then probably won't cause trouble for most people, but several cups daily or concentrated pu-erh supplements are more likely to add up. Please check with your pharmacist or doctor before mixing them regularly.

THEOPHYLLINE

3 of 3

Ephedrine, Hydroxyzine, Theophylline also falls under THEOPHYLLINE.

Theoretically, pu-erh tea might increase the levels and adverse effects of theophylline.
Pu-erh tea contains caffeine, which can increase theophylline levels.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combination product contains theophylline, a medication that your body handles in a fairly delicate way. Pu-erh tea naturally has caffeine, and caffeine is chemically similar to theophylline and uses the same liver pathways to be cleared. In theory, drinking pu-erh could slow how fast theophylline leaves your body, letting levels creep up. With theophylline that matters, since the safe range is narrow. Too much can cause jitteriness, a racing or pounding heart, nausea, trouble sleeping, or feeling shaky, which can overlap with the ephedrine in this product too.

This concern is theoretical, based on how the ingredients work rather than strong human studies. An occasional cup is unlikely to be a big deal, but heavy daily use could add up. Please check with your pharmacist or doctor before making it a routine.

Pu-erh tea is a fermented, aged form of tea made from the same plant as green and black tea.
