Red yeast rice contains varying levels of the drug lovastatin. Lovastatin can cause liver damage in some people. Some clinical research suggests that supplements containing red yeast rice might increase liver enzyme levels in some, but not all, participants. Cases of acute hepatitis have been associated with red yeast rice. Combining it with hepatotoxic drugs might further increase this risk.

Here's the concern with pairing red yeast rice and Avacopan (Tavneos). Both can be tough on the liver. Avacopan already carries a known risk of raising liver enzymes and, in some people, causing more serious liver injury. Red yeast rice naturally contains lovastatin, a statin that can also stress the liver. Stacking the two together could, in theory, add up to a bigger strain on your liver.

I'll be honest, this warning is based on how these substances behave, not on strong studies showing harm when they're combined. Still, since your doctor likely checks your liver labs while you're on Avacopan, it's worth flagging. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please check with your pharmacist or doctor before adding red yeast rice.

Here's the thing to know about pairing red yeast rice with Avacopan. Red yeast rice naturally contains lovastatin, a statin that your liver breaks down using an enzyme called CYP3A4. Avacopan can slow that same enzyme down a bit. So the concern isn't really about your Avacopan changing, it's that the lovastatin in the red yeast rice could clear out more slowly and build up in your blood.

If lovastatin levels climb, you're more likely to see typical statin side effects like muscle aches, weakness, or, rarely, more serious muscle breakdown that can stress the kidneys. This is a theoretical concern based on how these drugs behave, not something proven in studies, so try not to over-worry. Still, since red yeast rice doses vary wildly between brands, please check with your pharmacist or doctor before combining them.

What Is Avacopan Used For?

Brand name: Tavneos

Tavneos (avacopan) is used to treat severe active ANCA-associated vasculitis in adults — specifically the forms known as granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA). These are serious autoimmune conditions in which the immune system attacks small blood vessels, causing damage to the kidneys, lungs, and other organs.

Tavneos is used as an add-on treatment alongside standard therapies, including glucocorticoids (steroid medicines). It is not a replacement for steroids — it works together with them.
