Why this applies to Quinine
Quinine falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Red Yeast Rice interacts with.
Red yeast rice contains varying levels of the statin drug lovastatin, which is metabolized by CYP3A4. Combining red yeast rice with CYP3A4 inhibitors might increase serum levels of lovastatin from red yeast rice.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind with this pair: quinine can mildly slow down an enzyme in your liver called CYP3A4, and that same enzyme is what your body uses to clear the lovastatin naturally found in red yeast rice. So if you take both, the statin in your supplement could build up a bit higher than expected.
Why does that matter? More statin in your system means a greater chance of statin-type side effects, mainly muscle aches, soreness, or weakness, and rarely liver irritation. I'll be honest with you, this is a theoretical concern based on how these drugs behave, not something proven in studies, so don't panic. Still, since red yeast rice doses vary so much, it's worth running it past your pharmacist or doctor before combining them.

What Is Quinine Used For?
Brand name: Qualaquin
Quinine sulfate capsules (brands Qualaquin and Quinine Sulfate) are approved to treat uncomplicated Plasmodium falciparum malaria in adults. This includes strains of malaria that have become resistant to chloroquine.
Quinine sulfate capsules are not approved for severe or complicated malaria, for preventing malaria, or for treating or preventing nocturnal leg cramps. Using them for leg cramps carries serious risks that outweigh any potential benefit.
