Rhodiola + Azathioprine
Azathioprine brand names: Imuran, Azamune.
Rhodiola has a moderate interaction with Azathioprine. Theoretically, rhodiola use might interfere with immunosuppressive therapy.

Here is the interaction we found between these two, with the evidence behind it.
IMMUNOSUPPRESSANTS
Why this applies to Azathioprine
Azathioprine falls under IMMUNOSUPPRESSANTS — the group Rhodiola interacts with.
In vitro and animal research show that rhodiola has immunostimulatory effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thinking behind this one. Azathioprine works by dialing down your immune system, which is exactly what you want if you're protecting a transplanted kidney or calming down rheumatoid arthritis. Rhodiola seems to do the opposite in lab and animal studies, where it appears to rev up immune activity. So in theory, taking them together could push against each other and make your azathioprine work a little less well.
I want to be honest that this is a theoretical concern only, based on test-tube and animal research, not real patients. Still, because azathioprine is doing a serious job, I'd play it safe and check with your transplant team or rheumatologist before adding rhodiola. Please don't change your azathioprine on your own.

From our pharmacist’s overview on the Azathioprine page
“Azathioprine (Imuran) quiets an overactive immune system, which is why people take it after a transplant or for autoimmune disease, and it is also why I read this interaction list with real caution. Fully half of the 106 supplements on file, 53 of them, appear because they stimulate immunity and could blunt the very effect you depend on.”

About Rhodiola
What is Rhodiola?
- Fatigue and low energy
- Stress and coping (adaptogen)
- Mental performance and focus
- Mild depression
- Physical and exercise performance
Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. It is generally well tolerated for short-term use, but talk to your pharmacist or doctor before trying it, and avoid it during pregnancy and breastfeeding.
About Azathioprine
What Is Azathioprine Used For?
Brand names: Imuran, Azamune
Azathioprine is used alongside other medications to help prevent the body from rejecting a kidney transplant (renal homotransplantation). It is also used to treat active rheumatoid arthritis — reducing signs and symptoms in adults who have not responded well enough to other treatments.
Other pain relievers such as aspirin, NSAIDs, or low-dose steroids may be continued alongside azathioprine for rheumatoid arthritis. Combining azathioprine with disease-modifying antirheumatic drugs (DMARDs) has not been well studied and is not recommended.

Azathioprine Is Linked to Lower Levels of One Nutrient
Natural Medicines records Azathioprine as a medicine that can lower these. This is not an interaction with Rhodiola — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
