Rhodiola and Gabapentin

Here is the interaction we found between these two, with the evidence behind it.

Minor

CNS DEPRESSANTS

Why this applies to Gabapentin

Gabapentin falls under CNS DEPRESSANTS — the group Rhodiola interacts with.

Theoretically, rhodiola might increase the risk of adverse effects when taken with CNS depressants.
A review of adverse event reports in Poland identified cases of excessive sedation, myoclonus, hypotension, and hallucinations when rhodiola was taken with haloperidol, diazepam, or alprazolam.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: gabapentin already works by calming down activity in your nervous system, which is part of why it can leave you feeling drowsy, dizzy, or a little unsteady. Rhodiola is sometimes thought to add to that calming effect, so the worry is that pairing the two could make you feel more sleepy or foggy than usual.

I want to be honest about how shaky this concern is. It's mostly theoretical, based on a handful of anecdotal reports involving stronger sedatives like diazepam and alprazolam, not gabapentin itself. So this isn't something to panic over. Gabapentin isn't a prodrug, so Rhodiola isn't changing how your body activates it. Still, if you take both and notice extra drowsiness or wobbliness, give your pharmacist or doctor a quick call before making any changes.

From our pharmacist’s overview on the Gabapentin page

“Gabapentin (Neurontin, Gralise) calms overactive nerves, which means it already slows your nervous system down, and that is exactly what 78 of the 106 supplements on this list also do. So the dominant risk here is sedation stacked on sedation, extra drowsiness at best and dangerously slowed breathing at worst, and with seven entries ra…”

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. About Gabapentin

What Is Gabapentin Used For?

Brand names: Neurontin, Gralise

Gabapentin (brands include Neurontin, Relgaabi, Gabarone, Gralise, and Horizant) is primarily used to manage postherpetic neuralgia (PHN) — the nerve pain that lingers after a shingles outbreak — in adults. Neurontin, Relgaabi, Gabarone, and gabapentin capsules, tablets, and oral solution are also used as add-on therapy for partial onset seizures (a type of epilepsy) in adults and children aged 3 and older.

Horizant (gabapentin enacarbil extended-release tablet) has two specific approvals: treatment of moderate-to-severe primary restless legs syndrome (RLS) in adults, and management of postherpetic neuralgia in adults. Horizant is not recommended for people who need to sleep during the day and stay awake at night. Gralise is approved only for postherpetic neuralgia and cannot be substituted for other gabapentin products due to its different release profile.
