CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Lemborexant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rhodiola interacts with.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Your sleep aid lemborexant (Dayvigo) is cleared by a liver enzyme called CYP3A4. The thinking here is that rhodiola might slow that enzyme down a little, which could let lemborexant build up slightly in your body. If that happened, you might feel more of its usual effects the next day, things like grogginess, drowsiness, or trouble feeling fully alert in the morning.
The good news is this is a minor, mostly theoretical concern. It shows up in lab dishes, but a real human study found rhodiola did not actually slow the breakdown of a similar CYP3A4 drug. So a big problem is unlikely. Still, since both can make you sleepy, it is worth a quick chat with your pharmacist or doctor before combining them.

A review of adverse event reports in Poland identified cases of excessive sedation, myoclonus, hypotension, and hallucinations when rhodiola was taken with haloperidol, diazepam, or alprazolam.

Lemborexant (Dayvigo) already works by quieting brain activity to help you fall and stay asleep, so it can leave you feeling drowsy, groggy, or a little unsteady. Rhodiola is mostly known as an energizing herb, but there are a handful of reports where it seemed to add to the calming effects of other sedating medicines, with people feeling extra sleepy or low energy.
The concern here is mainly theoretical and based only on scattered case reports, not strong studies, and it's rated minor. Still, if you take both, just pay attention to whether you feel more sedated, foggy, or off balance the next morning. Nothing to panic over, but it's worth a quick chat with your pharmacist or doctor before combining them.
