These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Rooibos interacts with whole categories of medication. Chlorpheniramine Maleate, Dextromethorphan Hydrobromide falls into 2 of those categories, so it can interact with Rooibos in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 2Why this applies to Chlorpheniramine Maleate, Dextromethorphan Hydrobromide
Chlorpheniramine Maleate, Dextromethorphan Hydrobromide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Rooibos interacts with.
In vitro research shows that the methanol extract of rooibos leaves and stems inhibits CYP2D6 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo cough and cold product pairs an antihistamine (chlorpheniramine) with dextromethorphan, the cough suppressant. Both lean on a liver enzyme called CYP2D6 to be cleared from your body. Lab studies hint that rooibos may slow that enzyme down a bit, which in theory could let these medicines linger a little longer and feel stronger.
If that happened, you might notice more drowsiness, dry mouth, dizziness, or in the case of dextromethorphan, a foggy or spacey feeling. The honest truth is this is based only on test-tube research, not real people, so a normal cup or two of rooibos tea is very unlikely to cause trouble. Concentrated rooibos extracts are the bigger unknown. If you use those regularly, just check with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Chlorpheniramine Maleate, Dextromethorphan Hydrobromide also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP3A4 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo cold medicine has two active parts: chlorpheniramine (an antihistamine) and dextromethorphan (a cough suppressant). Both are partly broken down by a liver enzyme called CYP3A4. In a lab dish, a concentrated rooibos extract slowed that enzyme down. If the same thing happened in the body, your liver might clear these drugs a little slower, letting their levels nudge upward. That could mean more drowsiness, dizziness, dry mouth, or that foggy, sedated feeling.
Here's the honest part: this is only test-tube evidence, not something proven in people, and a regular cup of rooibos tea is very unlikely to cause trouble. Concentrated rooibos extracts are the bigger question mark. If you use those, just check with your pharmacist or doctor first.

From our pharmacist’s overview on the Chlorpheniramine Maleate, Dextromethorphan Hydrobromide page
“Grapefruit is the interaction I would flag first for anyone taking chlorpheniramine with dextromethorphan (Coricidin HBP Cough & Cold). It blocks the enzyme that clears these ingredients, so levels and side effects can climb.”
