Codeine, Papaverine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP2D6 substrates may increase the effects of CYP2D6 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems inhibits CYP2D6 enzyme activity.

Here's the thing about codeine: it's actually a prodrug. Your body uses an enzyme called CYP2D6 to convert codeine into morphine, which is what actually relieves your pain. So if rooibos blocks that enzyme, you'd expect less conversion to morphine, meaning weaker pain relief, not stronger. That direction is the opposite of what you might assume from the general note on file.

Now, the good news: this is based only on lab (test tube) research, not real people, so the evidence is weak. A cup or two of rooibos tea almost certainly won't change how your codeine works. I'd be a little more thoughtful with concentrated rooibos extracts. If your pain relief ever seems off, check in with your pharmacist or doctor.
