Donepezil falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP2D6 substrates may increase the effects of CYP2D6 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems inhibits CYP2D6 enzyme activity.

Here's the gist with donepezil and rooibos. Your liver uses an enzyme called CYP2D6 to help clear donepezil from your body. Lab studies (done in a test tube, not in people) suggest rooibos may slow that enzyme down a bit. In theory, that could let a little more donepezil build up than usual, which might nudge up its side effects like nausea, diarrhea, dizziness, a slower heartbeat, or trouble sleeping.

I want to be honest about how thin this evidence is. It comes from a concentrated rooibos extract in a lab, not from sipping the tea. A regular cup or two of rooibos tea is very unlikely to cause a problem. If you're using strong rooibos extracts, just give your pharmacist or doctor a heads-up first.

Here's the gist for donepezil (Aricept): your liver uses an enzyme called CYP3A4 to help clear this drug out of your system. In test-tube studies, concentrated rooibos extract seemed to slow that enzyme down. If that happened in the body, donepezil could build up a bit, which might mean stronger effects and more side effects like nausea, diarrhea, stomach cramps, dizziness, a slow heart rate, or trouble sleeping.

The important part: this is only theoretical, based on lab work, not real patients. A normal cup or two of rooibos tea is very unlikely to cause a problem. Concentrated extract supplements are a bigger unknown. If you're considering those, check with your pharmacist or doctor first.
