Ephedrine Sulfate, Hydroxyzine, Theophylline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP1A2 substrates may increase the effects of CYP1A2 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems inhibits CYP1A2 enzyme activity.

Of the three medicines in this combo, theophylline is the one to keep an eye on. Your liver clears theophylline using an enzyme called CYP1A2, and very early lab work suggests rooibos might slow that enzyme down a bit. If it does, theophylline could build up slightly, and that matters because theophylline has a narrow safety window. Too much can cause a racing heart, nausea, shakiness, trouble sleeping, or in larger excesses, dangerous heart rhythms.

Here's the reassuring part: this is only a theoretical concern from test-tube studies, not from people, and a normal cup of rooibos tea is unlikely to cause trouble. Concentrated rooibos extracts are the bigger question mark. Before adding those, check with your pharmacist or doctor.
