These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Rooibos interacts with whole categories of medication. Ibuprofen, Hydrocodone falls into 2 of those categories, so it can interact with Rooibos in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 2Why this applies to Ibuprofen, Hydrocodone
Ibuprofen, Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Rooibos interacts with.
In vitro research shows that the methanol extract of rooibos leaves and stems inhibits CYP2D6 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo pill pairs ibuprofen (an anti-inflammatory) with hydrocodone (an opioid for pain). The hydrocodone part matters here because your body uses an enzyme called CYP2D6 to turn it into hydromorphone, the form that actually relieves pain. Rooibos has been shown in test-tube studies to gently block that same enzyme. If it slowed that conversion, you might get slightly less pain relief from the hydrocodone.
Here's the reassuring part: this is based only on lab research, not real people, and a normal cup of rooibos tea is very unlikely to cause a noticeable problem. If you use concentrated rooibos extracts or feel your pain control changing, just check with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Ibuprofen, Hydrocodone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP3A4 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo pill, which pairs ibuprofen (an anti-inflammatory) with hydrocodone (an opioid for pain). The hydrocodone part is handled in your liver partly by an enzyme called CYP3A4. In test-tube studies, concentrated rooibos extract slowed that enzyme down. If that happened in the body, more hydrocodone could hang around, possibly leading to stronger effects like extra drowsiness, dizziness, constipation, or slowed breathing.
Honestly though, this is theoretical and based only on lab work, not real people. A cup or two of rooibos tea is very unlikely to cause trouble. I'd be more cautious with concentrated extract capsules. If you use those, check with your pharmacist or doctor first.

About Rooibos
What is Rooibos?
- Caffeine-free beverage
- Antioxidant support
- Heart health
- Blood sugar support
- Digestive upset
- General wellness
Rooibos is a caffeine-free herbal tea from South Africa that is rich in antioxidants and generally considered safe as a beverage. While lab and early studies suggest possible benefits for heart health, blood sugar, and inflammation, strong human evidence is lacking for most medical uses. Enjoy it as a tea, but talk to your pharmacist or doctor before using concentrated extracts for health reasons.
