Saw palmetto might have antiestrogenic effects. Theoretically, it might interfere with estrogens taken concomitantly.

If you take Bijuva (the combination of estradiol and progesterone) for menopause symptoms, here's the concern with saw palmetto. Some lab and early research suggests saw palmetto may have mild anti-estrogen activity, meaning it could push against the estrogen you're taking. In theory, that might make your hormone therapy work a little less well, so symptoms like hot flashes, night sweats, or vaginal dryness could creep back.
I want to be honest though: this is mostly theoretical and based on weaker studies, not strong human proof. It's not a dangerous combination. Still, since hormone therapy is something you take for a specific reason, it's worth a quick chat with your doctor or pharmacist before adding saw palmetto.
