Levorphanol is a strong opioid pain reliever, and like other opioids it slows down the brain and nervous system. That's where the concern comes in. Sceletium (kanna) seems to have its own mild calming, sedative quality, so pairing the two could stack those effects on top of each other. In plain terms, you might feel more drowsy, foggy, or dizzy than usual, and in theory your breathing could slow more than expected, which is the effect we worry about most with opioids.

To be fair, this caution is theoretical, based on how these substances work rather than on real cases with this exact combo. Still, since levorphanol is a powerful medicine, I'd run any plans to add sceletium past your pharmacist or doctor first.
