Why this applies to Ifosfamide

Ifosfamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schisandra interacts with.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the wrinkle with ifosfamide: it's actually a prodrug. Your liver uses an enzyme called CYP3A4 to switch it on into the active, cancer-fighting form (and also into some toxic byproducts). Schisandra tends to block that same enzyme, so in theory it could slow how ifosfamide gets activated. That muddies the picture, because it might mean less of the drug doing its job, while also shifting the balance of byproducts your body has to handle.

Honestly, the evidence here is mostly lab and animal data, not solid human studies, so this is more caution than alarm. But ifosfamide is serious chemo with real toxicity, so I would not add schisandra on your own. Please run it by your oncology team or pharmacist first.
