Aspirin, Caffeine, Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, inhibits cytochrome P450 (CYP) 2D6. Theoretically, schizonepeta might increase the effects and side effects of CYP2D6 substrates.
Some substrates of CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the wrinkle with this combo product: the hydrocodone part is actually a prodrug. Your liver uses an enzyme called CYP2D6 to turn some of it into hydromorphone, which gives a good chunk of the pain relief. Schizonepeta may gently block that same enzyme, but so far this is only seen in lab and animal studies, not in people.

If that blocking did happen, the surprising part is that you might get less pain relief, not more, since less of the drug gets activated. The aspirin and caffeine pieces aren't really affected. Honestly, the evidence here is weak, so I wouldn't panic. Still, if you're using this herb regularly, run it by your pharmacist or doctor first.

Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline, zileuton (Zyflo), zolmitriptan (Zomig), and others.

In this combination product, the piece that matters most for this interaction is the caffeine. Caffeine is cleared by a liver enzyme called CYP1A2. Lab and animal studies hint that a compound in Schizonepeta may slow that enzyme down. If it does, caffeine could hang around a bit longer, which might mean more jittery feelings, a racing heart, trouble sleeping, or an upset stomach.

I want to be honest about how thin this evidence is. It comes only from test tube and animal work, not from people, so the real-world effect may be small or none at all. The aspirin and hydrocodone parts are not driven by CYP1A2. If you notice extra jitteriness, please check with your pharmacist or doctor before changing anything.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the thing to know about this combo product (Damason-P). The piece that matters for Schizonepeta is the hydrocodone, an opioid pain reliever. Hydrocodone is processed in the body partly by an enzyme called CYP3A4. Lab and animal studies hint that a compound in Schizonepeta may speed that enzyme up, which could, in theory, clear hydrocodone a bit faster and leave you with weaker pain relief than expected.

I want to be honest about how thin this evidence is. It comes only from test-tube and animal work, not from people, so we really don't know if it matters in real life. The aspirin and caffeine parts aren't the concern here. Before mixing them, just run it by your pharmacist or doctor so they can keep an eye on your pain control.
