Sesame and Effient

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Why this applies to Effient

Effient falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Sesame interacts with.

Theoretically, sesame might increase the levels and clinical effects of CYP2C9 substrates.
In vitro, sesame inhibits CYP2C9. However, this interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about Effient (prasugrel): it's a prodrug, meaning it's inactive until your body converts it into its working form. So if sesame slows down a liver enzyme like CYP2C9, the direction of any effect isn't simple, and honestly the concern here is pretty mild to begin with.

This interaction is only theoretical. It comes from lab tests showing sesame can block CYP2C9, but it has never actually been seen in people. On top of that, Effient relies mostly on other enzymes (like CYP3A4 and CYP2B6) for activation, so CYP2C9 isn't its main pathway. Eating sesame in foods is very unlikely to matter. If you take concentrated sesame supplements, just mention it to your pharmacist or doctor to be safe.

  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Effient page

“Effient (prasugrel) keeps platelets from forming clots, usually after a stent or a heart attack, and I would not pair supplements with it casually. Roughly 200 ingredients on this list can thin the blood further and raise bleeding risk, including two of the five rated major.”

About Sesame

What is Sesame?

  • Heart and cholesterol health
  • Blood pressure support
  • Source of healthy fats and minerals
  • Skin and hair care (topical oil)
  • Antioxidant support
  • Cooking and nutrition

Sesame is a nutritious seed and oil widely used in cooking and traditional medicine, and it provides healthy fats, fiber, and antioxidant compounds called lignans. Some early research suggests sesame may modestly help with cholesterol and blood pressure, but the evidence is limited. Sesame is also a common, sometimes serious, food allergen, so people with seed allergies should be careful.

About Prasugrel

What Is Prasugrel Used For?

Brand name: Prasugrel

Prasugrel tablets (brand names Effient and Prasugrel) are used to lower the risk of dangerous blood clots — including clots that can block a stent — in people with acute coronary syndrome (ACS), which includes unstable angina (chest pain at rest) and heart attacks (both NSTEMI and STEMI types). It is used specifically in patients who are being treated with a procedure called percutaneous coronary intervention (PCI), in which a catheter is used to open a blocked coronary artery, often with a stent.

Effient has been shown to reduce the combined rate of cardiovascular death, nonfatal heart attack, or nonfatal stroke compared to clopidogrel — with the benefit driven mainly by fewer heart attacks.
