Tisotumab Vedotin-Tftv falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sulforaphane interacts with.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Tivdak (tisotumab vedotin). It's an antibody-drug conjugate, and the toxic payload it releases (a chemo agent called MMAE) is broken down in part by an enzyme called CYP3A4. In lab and animal studies, sulforaphane appears to slow that enzyme down. In theory, that could let a bit more of the drug's active payload hang around, which might mean stronger side effects like nausea, fatigue, hair loss, bleeding, nerve tingling, or eye problems.
I want to be honest about the evidence here: this is theoretical and has never been seen in actual people. Eating broccoli at the dinner table is very unlikely to matter. A concentrated sulforaphane supplement is a different story, so please run that by your oncology team before starting it.
