Swamp milkweed contains cardioactive steroids known as cardenolides that may have digitalis-like activity. Potassium depletion can increase the risk of cardenolide toxicity.

Here's the concern with this combination. Your medication contains hydrochlorothiazide, a water pill that can flush out potassium over time. Swamp Milkweed contains natural compounds that act a bit like the heart drug digoxin. When potassium runs low, those heart-acting compounds can become more toxic, which could lead to nausea, an irregular or slow heartbeat, dizziness, or weakness.

I want to be honest that this is a theoretical concern based on how these substances behave, not on solid human studies. Swamp Milkweed simply isn't a safe or proven supplement to take anyway, and it shouldn't be used as one. The deserpidine part isn't the issue here. Please check with your pharmacist or doctor before mixing the two.
