Why this applies to Esomeprazole

Esomeprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the short version on taking Sweet Annie with your esomeprazole (Nexium). Your body clears esomeprazole using a couple of liver enzymes, and one of them is CYP3A4. Sweet Annie's main compound, artemisinin, can tug on that enzyme, so in theory it could shift how much esomeprazole stays in your system. Depending on which way it pulls, you might get slightly weaker heartburn control, or a bit more medicine hanging around than usual.

The honest part: this is based only on lab and test-tube studies, and the herb seems to push the enzyme both ways, so we really don't know the real-world effect. Esomeprazole is also pretty forgiving. I wouldn't lose sleep over it, but mention it to your pharmacist or doctor before combining the two, especially if your reflux symptoms change.

What Is Esomeprazole Used For?

Brand name: Nexium

Esomeprazole treats problems caused by excess stomach acid. It's used to heal erosive esophagitis (damage to the esophagus from acid), control GERD (gastroesophageal reflux disease) symptoms like heartburn, and protect the stomach lining in people at risk for ulcers caused by NSAIDs (anti-inflammatory pain relievers like ibuprofen).

It's also used as part of a three-drug combination to wipe out H. pylori — a stomach bacteria linked to ulcers — and for people with rare conditions that cause the stomach to overproduce acid, such as Zollinger-Ellison syndrome.
