Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Terminalia interacts with.
In vitro research shows that Terminalia arjuna extract inhibits CYP2D6 enzymes and reduces CYP2D6 substrate metabolism.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this combo product: the hydrocodone piece is actually a prodrug. Your body uses an enzyme called CYP2D6 to turn it into its stronger painkilling form. Lab studies (test tube only, not people yet) suggest Terminalia arjuna may slow that same enzyme down. So in theory, less hydrocodone gets converted, which could mean you feel less pain relief than expected.
The good news is this is weak, early evidence, so I wouldn't lose sleep over it. The acetaminophen, caffeine, chlorpheniramine, and phenylephrine pieces aren't the main concern here. Still, if you take Terminalia regularly and your pain isn't well controlled, mention it to your pharmacist or doctor before changing anything.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product has a lot going on, but the piece that matters here is the hydrocodone, an opioid pain reliever and cough suppressant that your body partly processes through a liver enzyme called CYP3A4. In lab dish studies only, Terminalia arjuna seems to slow that enzyme down. In theory, that could let a bit more of the active medication linger in your system, nudging up effects like drowsiness, dizziness, constipation, or slowed breathing.
I want to be honest though: this is based on test-tube research, not real people, so we genuinely don't know if it matters in everyday life. The caffeine and acetaminophen pieces also lean on the liver. I wouldn't panic, but since this product already carries an opioid, please run it by your pharmacist or doctor before combining.
