Alogliptin, Pioglitazone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the gist with Oseni (which combines alogliptin and pioglitazone) and Terminalia arjuna. The pioglitazone part is partly cleared by a liver enzyme called CYP3A4. Lab studies suggest Terminalia may slow that enzyme down, which could in theory let a bit more pioglitazone build up in your system. If that happened, you might see stronger effects, things like low blood sugar (shakiness, sweating, dizziness), some fluid retention or swelling, or weight gain.

That said, this is based only on test-tube research, not on real people, so the actual risk looks small and unproven. I wouldn't lose sleep over it, but since both Terminalia and Oseni affect the heart and blood sugar, please check in with your pharmacist or doctor before combining them.

Alogliptin, Pioglitazone also falls under ANTIDIABETES DRUGS.

Theoretically, concomitant use of Terminalia bellirica or Terminalia chebula with antidiabetes drugs could affect blood sugar control and increase the risk of hypoglycemia.
Animal and in vitro research shows that Terminalia bellirica and Terminalia chebula fruit and seed extract have hypoglycemic effects.

Oseni combines two diabetes medicines, alogliptin and pioglitazone, that both work to lower your blood sugar. Here's the thing about Terminalia: some lab and animal studies hint that certain species (like Terminalia bellirica and chebula) may have their own blood-sugar-lowering effect. If that holds true in people, stacking it on top of your Oseni could nudge your sugar down a little too far.

I want to be honest with you: this is theoretical and based on early research, not solid human studies. Still, if you decide to try it, keep an eye out for low blood sugar signs (shakiness, sweating, dizziness, sudden hunger) and check your levels more closely at first.
