Aspirin, Aluminum Hydroxide, Codeine Phosphate, Magnesium Hydroxide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2D6 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2D6 enzymes and reduces CYP2D6 substrate metabolism.

The piece worth focusing on here is the codeine in this combination product. Codeine itself doesn't do much for pain until your body converts it into morphine, and the enzyme that does that conversion is CYP2D6. Lab studies suggest Terminalia arjuna may block CYP2D6. Because codeine is a prodrug that depends on this enzyme to work, slowing the enzyme could mean less morphine is formed, which may translate into weaker pain relief rather than stronger effects.

Keep in mind this is only test-tube evidence, so we don't truly know how meaningful it is in real people. The aspirin, aluminum hydroxide, and magnesium hydroxide aren't affected by this. If you take Terminalia regularly and rely on this medicine for pain, check in with your pharmacist or doctor before mixing them.

Theoretically, concomitant use of Terminalia arjuna with anticoagulant or antiplatelet drugs may increase the risk of bleeding in some patients.
In vitro, Terminalia arjuna bark extract inhibits platelet aggregation, decreases platelet activation, and shows antithrombotic properties.

This combination product contains aspirin, which already thins the blood by blocking platelets from sticking together. Terminalia arjuna may do something similar. In lab studies (test tube, not people), its bark extract slowed platelets and showed clot-fighting activity. So in theory, pairing the two could nudge your bleeding risk a little higher. Things to watch for would be easier bruising, nosebleeds, bleeding gums, or a cut that takes longer to stop.

Keep in mind this is theoretical and based only on early lab work, so it's far from proven. The aluminum and magnesium antacids and the codeine part aren't really involved here. Still, since you're already on aspirin, it's worth a quick chat with your pharmacist or doctor before adding Terminalia, especially if you bleed or bruise easily.
