Atazanavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Your Evotaz combo (atazanavir plus cobicistat) leans heavily on the liver enzyme CYP3A4. In fact, cobicistat is in there on purpose to block that enzyme and keep atazanavir levels up. Lab studies hint that Terminalia arjuna may also slow CYP3A4. In theory, layering it on top could nudge atazanavir levels even higher, which might mean more of its usual side effects, like yellowing of the skin or eyes (jaundice), nausea, or rhythm changes in the heart.

I want to be honest: this is only test-tube data, not proven in people, so it may not amount to much. Still, since HIV therapy is something you really don't want to throw off, please run Terminalia past your pharmacist or HIV provider before starting it.

What Is Atazanavir and Cobicistat Used For?

Brand name: Evotaz

Evotaz (atazanavir/cobicistat) is used to treat HIV-1 infection. It must always be taken together with other antiretroviral medicines — it is not a complete regimen on its own. It is approved for adults and for pediatric patients who weigh at least 35 kg.

In patients who have been on HIV treatment before (treatment-experienced), how well Evotaz works depends on how much resistance to protease inhibitors the virus has already developed. Your prescriber will review this before starting you on Evotaz.
