Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down and clear bedaquiline. Lab studies suggest that Terminalia arjuna may slow that enzyme down. In theory, that could let bedaquiline build up a bit higher than usual in your system, which may increase the chance of side effects.

With bedaquiline, the side effect I'd watch most closely is its effect on heart rhythm (a QT prolongation that shows up on an EKG), plus things like nausea or higher liver enzymes. I want to be honest though: this concern comes only from test-tube research, not from people, so it's theoretical. Since TB treatment is something you can't afford to mess up, please don't start Terminalia without first checking with your TB doctor or pharmacist.

Terminalia is a group of traditional Ayurvedic tree species (most notably Terminalia arjuna) used for heart, digestive, and general wellness purposes. About Bedaquiline

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
