Bivalirudin falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Terminalia interacts with.

Theoretically, concomitant use of Terminalia arjuna with anticoagulant or antiplatelet drugs may increase the risk of bleeding in some patients.
In vitro, Terminalia arjuna bark extract inhibits platelet aggregation, decreases platelet activation, and shows antithrombotic properties.

Here's the simple version: Bivalirudin is a powerful blood thinner given through an IV, usually in a hospital during heart procedures like angioplasty or stent placement. Its whole job is to stop your blood from clotting. Terminalia (especially Terminalia arjuna) appears to make platelets a little less sticky too, so in theory stacking the two could nudge your bleeding risk a bit higher. That could mean more bruising, oozing at the IV or catheter site, or harder-to-stop bleeding.

I want to be honest about how strong this is: the concern comes only from lab and test-tube studies, not from real patients, so it's theoretical. Still, since bivalirudin is given in a closely watched hospital setting, please tell your care team about any Terminalia or herbal supplements you take so they can keep an eye on you.
