Diphenhydramine Hydrochloride, Naproxen Sodium falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Terminalia interacts with.
In vitro research shows that Terminalia arjuna extract inhibits CYP2C9 enzymes and reduces CYP2C9 substrate metabolism.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Aleve PM, which pairs naproxen (a pain reliever and anti-inflammatory) with diphenhydramine (a sedating antihistamine). The naproxen half is partly broken down by a liver enzyme called CYP2C9. Lab studies (not human ones) hint that Terminalia arjuna may slow that enzyme down, which in theory could let naproxen build up a bit and linger longer.
If that happened, you might notice more of naproxen's usual effects, like stomach upset, heartburn, or a higher chance of stomach irritation. I want to stress this is theoretical and based only on test-tube data, so it's not a strong concern. Still, if you regularly take both, it's worth a quick chat with your pharmacist or doctor.

ANTICOAGULANT/ANTIPLATELET DRUGS

In vitro, Terminalia arjuna bark extract inhibits platelet aggregation, decreases platelet activation, and shows antithrombotic properties.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Aleve PM combines naproxen (an NSAID pain reliever) with diphenhydramine (an antihistamine that makes you sleepy). The piece that matters here is the naproxen. Like other NSAIDs, naproxen can thin the blood a little and irritate the stomach lining, which raises bleeding risk on its own.
Terminalia (especially Terminalia arjuna) has shown, mostly in lab and test-tube studies, that it may keep platelets from clumping together. Stack that mild blood-thinning effect on top of naproxen and, in theory, you could see more bruising, nosebleeds, or stomach upset and bleeding. This is theoretical and graded on weak evidence, so don't panic, but if you take both regularly, watch for unusual bruising or dark stools and check with your pharmacist or doctor first.
