Doxepin falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2D6 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2D6 enzymes and reduces CYP2D6 substrate metabolism.

Here's the gist: your body relies on a liver enzyme called CYP2D6 to help clear doxepin out of your system. Lab studies suggest that Terminalia arjuna may slow that enzyme down. If it does, doxepin could build up a bit more than usual, which may nudge up its sedating, anticholinergic-type side effects, things like extra drowsiness, dry mouth, dizziness, constipation, or a faster heartbeat.

Now, the honest part: this is only seen in test-tube research, not in real people, so we don't know if it actually matters at the doses you'd take. I wouldn't panic. But since both can make you sleepy, it's worth a quick chat with your pharmacist or doctor before pairing them, especially if you notice you feel more groggy than normal.
