Iloprost falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Terminalia interacts with.

Theoretically, concomitant use of Terminalia arjuna with anticoagulant or antiplatelet drugs may increase the risk of bleeding in some patients.
In vitro, Terminalia arjuna bark extract inhibits platelet aggregation, decreases platelet activation, and shows antithrombotic properties.

Here's the thing to know about combining Terminalia (often Terminalia arjuna) with iloprost (Ventavis). Iloprost relaxes blood vessels to ease pulmonary hypertension, but it also has a known side effect of slowing down platelets, which are the cells that help your blood clot. In lab studies, Terminalia appears to do something similar, gently discouraging platelets from sticking together.

So the worry is additive: stacking the two could nudge you toward easier bruising, nosebleeds, or bleeding that's slower to stop. I want to be honest though, this is theoretical and based only on test-tube research, not real patient cases, so it may not amount to much. Still, since iloprost already affects bleeding, please check with your pharmacist or doctor before adding Terminalia.
