Perampanel falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2C9 enzymes and reduces CYP2C9 substrate metabolism.

Here's the gist for you. Your body clears perampanel partly through a liver enzyme called CYP2C9. Lab studies (done in test tubes, not in people) suggest that Terminalia arjuna may slow that enzyme down. If it does, perampanel could clear out more slowly and build up a bit, which might make its effects and side effects stronger.

With perampanel that could mean more dizziness, drowsiness, trouble with balance, or mood and irritability changes. I want to be honest though: this is only a theoretical concern based on early lab work, not real-world cases. Concentrated supplement doses are more of a question mark than an occasional tea. Before mixing the two, please run it by your pharmacist or doctor.

Your seizure medicine Fycompa (perampanel) gets cleared from your body mainly by a liver enzyme called CYP3A4. Terminalia, especially Terminalia arjuna, may slow that enzyme down. In theory, that could let perampanel build up a bit higher than usual. Since perampanel is active on its own (not a prodrug that needs switching on), more of it in your system could mean stronger effects and more side effects, like dizziness, drowsiness, trouble with balance, or mood changes such as irritability or anger.

I want to keep this in perspective though: this concern comes only from lab (test tube) research, not from studies in people, so we honestly don't know how much it matters in real life. Please don't start or stop anything on your own. Just check with your pharmacist or doctor first.

Terminalia is a group of traditional Ayurvedic tree species (most notably Terminalia arjuna) used for heart, digestive, and general wellness purposes. Some small studies suggest possible benefits for heart and cholesterol health, but the evidence is limited and not strong enough to recommend it as a treatment. What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
