Segesterone Acetate, Ethinyl Estradiol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Annovera's two hormones, segesterone acetate and ethinyl estradiol, are both cleared from your body partly by a liver enzyme called CYP3A4. In a lab dish, Terminalia arjuna seemed to slow that enzyme down. If that holds true in real people, your hormone levels could nudge a bit higher, which might mean more of the usual estrogen-type side effects like nausea, breast tenderness, headaches, or bloating.

I'll be honest with you: this is only a theoretical concern based on test-tube work, not on actual patients, so I wouldn't lose sleep over it. Still, since this is your birth control, it's worth a quick chat with your pharmacist or doctor before adding a concentrated Terminalia supplement, so you can keep an eye out for any new symptoms.
