This combination pill (Fiorinal with Codeine) already has two ingredients that calm the brain down: butalbital, a barbiturate, and codeine, an opioid. Both can make you drowsy, foggy, or slow your breathing if doses run high. The concern here is simple overlap. Animal studies hint that berberine, the active part of tree turmeric, may have its own mild sedating effect. Stacking that on top of these depressants could, in theory, add to the sleepiness, dizziness, or slowed thinking.
Keep in mind this is based only on lab and animal research, so it's a soft, theoretical caution rather than a proven problem. Still, I'd mention any berberine or tree turmeric supplement to your pharmacist or doctor before pairing it with this medication.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, can inhibit CYP2D6.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this combo product: the codeine part is a prodrug. Your body has to convert codeine into morphine using a liver enzyme called CYP2D6 before you get much pain relief. The berberine in tree turmeric can slow that same enzyme down. So in theory, less codeine gets turned into morphine, which could mean weaker pain control from this medication.
The good news is this is mostly based on lab work and limited human data, so it's a 'possible' concern, not a sure thing. Food amounts of turmeric spice aren't the issue; concentrated berberine supplements are. The aspirin, butalbital, and caffeine pieces aren't really affected here. Before adding tree turmeric, check with your pharmacist or doctor.

In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, moderately inhibits CYP3A4.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
This combination pill has a few moving parts, but the piece that matters here is butalbital, the barbiturate that helps with tension headaches. Tree turmeric's main compound, berberine, can gently slow down a liver enzyme (CYP3A4) that helps break butalbital down. When that clearing slows, butalbital can build up a bit more than expected, which may leave you feeling extra drowsy, dizzy, foggy, or unsteady.
The codeine side is a little different since codeine relies more on a separate enzyme to work, so the main concern stays with that sedating, sleepy effect. This is a moderate, mostly theoretical concern, but since it's a combo product, please check with your pharmacist or doctor before pairing them rather than adjusting anything yourself.

The piece of Fiorinal with Codeine that matters here is the aspirin. Aspirin already thins the blood by keeping platelets from clumping together. Lab and animal studies hint that the berberine in tree turmeric may do something similar, blocking platelets too. Stacked on top of aspirin, that could add up and make bleeding or bruising a bit more likely, things like nosebleeds, easy bruising, or bleeding gums.
I want to be honest about the strength of this: it comes from test tube and animal research, not solid human studies, so it's more of a theoretical caution than a proven problem. Still, if you take this combo, especially around surgery or dental work, let your pharmacist or doctor know before adding tree turmeric.
