Here's the simple version. Synalgos DC contains dihydrocodeine, an opioid pain reliever that can make you drowsy and slow your breathing. The concern with tree turmeric (berberine) is that, in animal studies, it showed mild sedative effects of its own. So the worry is mostly about adding up: taking both could leave you more tired, lightheaded, or foggy than the medicine alone would.
I want to be honest about the strength of this. It comes from lab and animal research, not solid human studies, so it's theoretical and not a sure thing. Still, since this combo already carries codeine, caffeine, and aspirin, it's worth a quick chat with your pharmacist or doctor before adding berberine, especially if you already feel sleepy on it.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, can inhibit CYP2D6.

Here's the wrinkle with Synalgos DC: the dihydrocodeine part is a prodrug. Your liver uses an enzyme called CYP2D6 to convert it into the stronger painkilling form. The berberine in tree turmeric appears to slow that same enzyme down. So instead of building up dihydrocodeine, blocking CYP2D6 may mean less of it gets activated, which could leave you with weaker pain relief than you expect. The aspirin and caffeine pieces aren't really affected by this.
To be fair, this is mostly theoretical and based on lab work plus some early human data, so it may not amount to much for everyone. Still, if your pain isn't well controlled while taking both, don't adjust anything on your own. Just check in with your pharmacist or doctor first.

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to know about this combo (Synalgos-DC). The berberine in tree turmeric can mildly slow down a liver enzyme called CYP3A4, which helps process some medicines. Caffeine, the dihydrocodeine, and parts of how your body handles these ingredients can lean on that pathway. So in theory, holding these together could let the medicine build up a little more than usual.
What might that look like? With the dihydrocodeine portion, you could feel more drowsy, dizzy, constipated, or notice slower breathing. The caffeine could feel a bit stronger too, like jitters or trouble sleeping. This is mostly theoretical and the effect is moderate, so I wouldn't panic, but please run it by your pharmacist or doctor before pairing them, especially at concentrated supplement doses.

The piece of this combo pill that matters here is the aspirin. Aspirin already thins your blood by keeping platelets from clumping together. In real life that might look like easier bruising, nosebleeds, bleeding gums, or a cut that takes longer to stop.
I want to be honest about the evidence: this is theoretical and comes from test-tube and animal work, not solid human studies, so for most people it's a mild concern rather than a red flag.
