Riociguat falls under ANTIHYPERTENSIVE DRUGS — the group Tree Turmeric interacts with.

Theoretically, taking tree turmeric along with antihypertensive drugs might have additive effects and increase the risk of hypotension.
Animal research suggests that berberine, a constituent of tree turmeric, can have hypotensive effects. Also, a meta-analysis of clinical research suggests that taking berberine in combination with amlodipine (Norvasc) can lower systolic and diastolic blood pressure when compared with taking amlodipine alone.

Here's the thing to keep in mind: riociguat (Adempas) already lowers blood pressure as part of how it works, since it relaxes and widens blood vessels in the lungs and the rest of the body. Tree turmeric, thanks to its berberine, may also nudge blood pressure downward. Stack the two together and you could see an additive drop, meaning more dizziness, lightheadedness, headache, or even fainting, especially when you stand up quickly.

I'll be honest with you, this is mostly a theoretical concern based on animal studies and some research with other blood pressure drugs, not direct testing with riociguat. Still, since low blood pressure is a known issue with this medication, it's worth a quick chat with your pharmacist or doctor before adding tree turmeric.

Theoretically, tree turmeric might increase the levels and clinical effects of drugs that are substrates of CYP3A4.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, moderately inhibits CYP3A4.

Here's the gist if you're taking Adempas (riociguat) and thinking about tree turmeric. Riociguat relies on an enzyme called CYP3A4 to help break it down. The berberine in tree turmeric appears to slow that enzyme down a bit. When the cleanup crew slows, more riociguat can hang around in your blood than expected.

Because riociguat works by relaxing blood vessels, higher levels could push your blood pressure too low. Watch for dizziness, lightheadedness, headache, feeling faint, or an upset stomach. This is mostly a theoretical and lab-based concern rather than something proven with riociguat itself, but since low blood pressure is a real risk with this drug, I'd play it safe. Please check with your pharmacist or doctor before combining the two.

About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold as Adempas and under the generic name Riociguat — are used to treat adults with two types of pulmonary hypertension (high blood pressure in the lungs).

The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots in the lung arteries, either after surgery or when surgery is not an option. The second is pulmonary arterial hypertension (PAH), where the arteries carrying blood to the lungs are narrowed or blocked. In both conditions, riociguat helps improve exercise ability and functional status, and in PAH it also helps slow the disease from getting worse.
