Here's the thing to know about pairing tree turmeric with Koselugo (selumetinib). Your child's body relies on a liver enzyme called CYP3A4 to clear selumetinib out of the system. The berberine in tree turmeric appears to slow that enzyme down a bit, so the medicine could clear out more slowly and build up higher than expected.

Selumetinib is an active drug (not a prodrug), so more of it in the blood usually means a stronger effect and more side effects, things like diarrhea, nausea, mouth sores, rash, tiredness, or, less commonly, heart or eye changes that this drug is watched for. The interaction is mostly theoretical, but selumetinib matters too much to guess with. Please loop in your oncology team or pharmacist before adding any berberine supplement.

Here's the honest picture on combining tree turmeric (berberine) with Koselugo. The interaction we have on file is really about berberine possibly thinning the blood a little, based only on lab and animal studies showing it may slow platelets from clumping. That's very early evidence, not something proven in people, so I wouldn't lose sleep over it.

What makes this worth a mention is that selumetinib itself can sometimes cause bleeding or bruising as a side effect. So the theoretical worry is that stacking the two could nudge the bleeding risk up a bit. Watch for easy bruising, nosebleeds, or bleeding gums. Before adding tree turmeric, please run it by your child's oncology team or pharmacist first.

What Is Selumetinib Used For?

Brand name: Koselugo

Koselugo (selumetinib) is approved for adults and children 1 year of age and older who have neurofibromatosis type 1 (NF1) — a genetic condition that causes tumors to grow on nerve tissue — when they also have symptomatic, inoperable plexiform neurofibromas (large, deep nerve tumors that cannot be surgically removed and are causing problems).

Both the Koselugo capsule and Koselugo oral granule formulations are approved for this use. The oral granule form is intended for patients who have difficulty swallowing whole capsules.
