Sirolimus (albumin bound) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tree Turmeric interacts with.

Theoretically, tree turmeric might increase the levels and clinical effects of drugs that are substrates of CYP3A4.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, moderately inhibits CYP3A4.

Here's the key thing with Fyarro (albumin-bound sirolimus): your body relies on a liver enzyme called CYP3A4 to break it down and clear it out. The berberine in tree turmeric appears to slow that enzyme down. When that cleanup slows, sirolimus can pile up in your blood, and higher levels can mean more side effects, like mouth sores, low blood cell counts, infections, high blood sugar or cholesterol, and swelling.

Sirolimus isn't a prodrug, so this goes in one direction: more drug, stronger effect. It's also a medication where small changes in level matter. The interaction is rated probable, though the human evidence is still early. Please don't change anything on your own. Loop in your oncology team or pharmacist before adding tree turmeric or berberine.
