Avipaxin contains three active ingredients in a proprietary blend. Acetyl-L-carnitine HCl is an amino acid compound thought to support brain energy and neurotransmitter function.
Alpha-glycerylphosphorylcholine (alpha-GPC) is a naturally occurring compound involved in cell membrane and brain chemistry. Huperzia serrata, also called toothed clubmoss, is a plant that contains huperzine A, an alkaloid that acts on acetylcholine, a key neurotransmitter for memory and cognition.
The capsule also contains microcrystalline cellulose, magnesium stearate, and silica as inactive ingredients.

Acetyl-L-carnitine is rated possibly effective for age-related cognitive decline, Alzheimer disease, alcohol use disorder, diabetic neuropathy, and depression based on the evidence we hold. Alpha-glycerylphosphorylcholine is possibly effective for Alzheimer disease, but evidence is insufficient for cognitive function, brain tumors, vascular dementia, and stroke.

How safe is it?
Well-documented data
Acetyl-L-carnitine is generally well tolerated in the short term, though long-term safety isn't fully established. Common side effects include agitation, dry mouth, headache, insomnia, and reduced appetite.
Some people report a fishy odor of urine, breath, or sweat. Gastrointestinal upset (nausea, vomiting, diarrhea, constipation) can occur but typically no more often than with placebo.
Alpha-glycerylphosphorylcholine is generally well tolerated short-term, but long-term safety is not fully established. Rare side effects include dizziness, headache, insomnia, and nausea.
One concern: oral use for 2 months or longer has been associated with an elevated stroke risk compared to non-users or shorter-term use. Huperzia serrata may cause dizziness, nausea, and sweating; product quality varies widely.
Safety information is insufficient; pregnancy and breastfeeding use is not recommended.

Acetyl-L-carnitine also affects blood thinners. It may increase the anticoagulant effects of warfarin (Coumadin) and acenocoumarol (Sintrom), raising your bleeding risk.
It can also reduce how well thyroid hormone replacement works by blocking thyroid hormone from entering your cells.
Huperzia serrata, another ingredient, contains huperzine A, which acts like a cholinergic drug — a type that affects nerve signaling. It may clash with other cholinergic medications (like donepezil for Alzheimer disease) and anticholinergic drugs, either amplifying side effects or reducing effectiveness.
Alpha-glycerylphosphorylcholine has only one documented interaction: it may weaken the effects of scopolamine (Transderm Scop) patches used for motion sickness.

Huperzia serrata
Anticholinergic Drugs
In animal models, toothed clubmoss and huperzine A, an active constituent of toothed clubmoss, reversed cognitive deficits induced by scopolamine. Theoretically, concurrent use of anticholinergic drugs and toothed clubmoss might decrease the effectiveness of toothed clubmoss or the anticholinergic drug.
Some anticholinergic drugs include atropine, benztropine (Cogentin), biperiden (Akineton), procyclidine (Kemadrin), and trihexyphenidyl (Artane).
Cholinergic Drugs
Huperzine A, a constituent of toothed clubmoss, has demonstrated acetylcholinesterase inhibitory properties. Theoretically, concurrent use of toothed clubmoss with cholinergic drugs might have additive effects and increase the risk of cholinergic side effects.
Cholinergic drugs include bethanechol (Urecholine), donepezil (Aricept), echothiophate (Phospholine Iodide), edrophonium (Enlon, Reversol, Tensilon), neostigmine (Prostigmin), physostigmine (Antilirium), pyridostigmine (Mestinon, Regonol), succinylcholine (Anectine, Quelicin), and tacrine (Cognex).
Acetyl-L-Carnitine HCl
Acenocoumarol (Sintrom)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine, the parent compound of acetyl-L-carnitine, might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant that is similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation when L-carnitine was taken with acenocoumarol. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product. It is unclear if such an interaction would also occur with acetyl-L-carnitine.
Serotonergic Drugs
Theoretically, acetyl-L-carnitine might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders, when taken with serotonergic drugs.
Animal research shows that acetyl-L-carnitine can increase levels of serotonin in the brain.
Thyroid Hormone
Theoretically, acetyl-L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism. It is unclear if such an interaction would occur with acetyl-L-carnitine.
Warfarin (Coumadin)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine, the parent compound of acetyl-L-carnitine, might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with acetyl-L-carnitine and warfarin.
Alpha-Glycerylphosphorylcholine
Scopolamine (Transderm Scop)
Theoretically, alpha-GPC might decrease the effects of scopolamine.
A small clinical study shows that alpha-GPC can partially counteract the attention and memory impairment effects caused by scopolamine given intramuscularly. Whether alpha-GPC can decrease the beneficial anti-motion sickness effects of the scopolamine patch (Transderm Scop) is unclear.
